The Palm Beach County School Board Policy for authorization of medication for students during the school day is as follows:
1. Medication to be given to a student during the school day must be arranged through the Main Office and will be kept in the office. Requests from a parent or guardian for their child to receive medication during the school hours must be made by submitting to the school a completed "Physicians Authorization of Medication for a School Student" form (available in the office) which details the name of the drug, dosage, and time interval for the medication which the student is to receive. This form requires a physician's signature and contains a statement of parental permission, which must be signed by the parent/guardian. A separate "Authorization of Medication for a Student at School" form must be submitted for each medication a student is to receive.
2. The medication which has been prescribed by the physician must be brought to the school in a container appropriately labeled by the pharmacy or by the physician. Non-prescription medications cannot be taken by students without a physician authorization form too.
