AC 2 - 2012-2014 School Health Services Plan


The School Health Services plan is prepared and reviewed by the Department of Health (DOH) and the Department of Education (DOE) to clearly identify each of the statutory requirements that relate to mandated basic student health services in all public schools.

“Column 1 – Statutory Requirements” and “Column 2 – Program Standards” are mandatory to this plan and established by DOH and DOE to establish the primary requirements and mandates for school health.  Chapter 64F-6.002 F.A.C. requires the plan to be completed biennially.  Therefore, you are receiving the 2012-2014 plan at this time.

During this biennial time frame, the School Health Services plan is submitted to local school health agencies to provide strategies that will be used to implement the services mandated by law in “Column 3 – Local Agency Responsible” and “Column 4 – Local Implementation Strategy & Activities”. 

Working together, the County Health Department (CHD), School District (LEA), Health Care District (HCD), and School Health Advisory Committee (SHAC) identify and determine the responsibilities for providing the school health services described by DOH and DOE.  This is a collaborative process to define the activities and services provided based upon each agency staffing models, funding sources, and community partnerships.


I recommend that the School Board approve the 2012-2014 School Health Services Plan, and authorize the Superintendent or designee to sign all documents related to this plan required for implementation.
